Find the Long Live Halloween NPC at Rudis (X: 102, Y: 93) and receive the quest.

Quest requirements:

1x Jack O' Lantern Final (Lv.191) - from Halloween Sorcerer's Apprentice Quest

1x NeoPumpkinGirl (Lv.185)

50x Special Artreia Coin (M) - Can be purchased from Free Battle Zone Merchant

Quest reward: 

1x Jack O' Lantern D (Lv.202)

This quest has no time limit and can be completed at any time after the halloween event.
